I have a copy of his Utah Death certificate, # 2503486. It lists his name as "Charley A. Martindale", his PDOB as Clark County, Wisconsin on 21 April 1887. His occupation is listed as "Combination Cook" and showed him as unemployed. His address was listed as 49 West Second South, Salt Lake City, UT. He died 16 May 1925. If I read the certificate right, his actual place of death is Echo, Summit, Utah, tho he is buried in Coalville Cemetary, Coalville, UT. Cause of death is listed as "Struck by railroad engine 2257 UPRRCo. Fractured skull and neck-broken-internal injuries.
